Dear Portage Central Marching Band (PCMB) Families 2019!!

It is so hard to believe we will be starting to get the season rolling soon. We will be announcing the long awaited name of the show on May 13th, so if you are a new student joining us keep an eye on the PC Band facebook/Instagram or in your email to find out details! You will love it!

Below are all of the details on how to get your son or daughter officially registered for marching band. We know you have already chosen the “Symphonic/Concert Band” (Marching Band) class in your schedules, but we require this additional registration step to secure your son/daughter a spot in the marching band.

Please do not hesitate to email with questions. We are glad to help with anything you need. We have also streamlined our forms in google docs and our new online payment site to make things easier for everyone.

It is mandatory that at least one parent/guardian be present for each student in order to get all of the information needed and ask questions.

Registration and Medical Form Checklist 2019

Please use this as a guide to make sure your son/daughter is registered for Marching Band! We must receive all portions of the registration/medical form/Payment to confirm their participation and guarantee a spot in the show.

The Medical Form submitted (through DocuSign), payment and the Google Registration Form are due by Wednesday May 22nd

1-PC Marching Band Registration is to be completed in the following google form ASAP so we can get you medical form emailed to you: Time to complete this portion is about 5 minutes. - https://docs.google.com/forms/d/e/1FAIpQLSc4EfKXkZm4lZu44zYmIrMLKgA0BIhuL_-jsE3Ca-5zdo_GGg/viewform

2-The Marching Band Fee is $300 per student and is due in full by May 22nd or can be paid in 2 payments (1st - $150 due by May 22nd, 2nd - $150 due by June 12th). You may pay online at our payment website: www.pcbands.ecwid.com or by checks made out to "PCHS Bands" and sent to "Portage Central Band Boosters - P.O. Box 1592 - Portage, MI 49081"

3-PC Marching Band Medical Form All medical forms must be completed electronically via DocuSign. A link to the form will be emailed from Mrs. Flynn as soon as you have filled out the google form registration. This is the 3rd year that we are using this online medical form and we have utilized this entirely secure site to make all of our jobs easier! It allows us to load a pdf of our medical form to the site and put in text boxes for you to fill in information. As part of using this site, we will be emailing it out individually to one parent email per student, so please search your email accounts for an email from DocuSign, sent by Mrs. Flynn. If you do not receive the form by May 7th, Please email Mrs. Flynn at rflynn@portageps.org as she will email another copy to the email of your choice. For new marching band members, we will wait until you have submitted the google form registration before sending you the medical form, so that we have your correct email address.

4-The medical form also, requires a copy of the front AND back of your medical insurance card for your son/daughter. There are 4 paper clips in this section, allowing you to take a picture of the front and back of your card or scan them and upload them. This item is required in order to submit the form, so if you have problems, please email Mrs. Flynn at - rflynn@portageps.org

5-The medical form is a very important piece for marching band and the on-line form will not allow you to leave any section blank. The time to complete this will be lengthy, so get started on it now! There is an option in the top right in the menu to “Finish Later”, so you can revisit it and once it is complete, you must submit it before May 22nd!

*Additionally, we DO NOT NEED a physical/sports form attached and physical/sports forms will not stand as a substitute for the marching band medical form. Thank you so much for your attention to the details and making our jobs easier!!

We have also scheduled a mandatory meeting for new marching band parents on May 22nd and for returning parents on May 29th both at 7pm in the CHS auditorium
